All pre-employment screening will be performed pursuant to and in compliance with Federal and State laws. After the application is submitted, the hiring manager will make a determination as to their interest in interviewing the applicant for a position. After the interviewing process, a determination will be made whether to make an offer of employment.

A facsimile (FAX) or copy of this authorization shall be as valid as the original. Permission is granted for information to be released by any state agency or from any Consumer Credit Reporting Agency. The information obtained from these reports will only be used for assistance in the hiring decision and will not be disclosed to any other person. I agree and understand any offer of employment I may receive is contingent upon mu successful completion of the Company's pre-employment screening.
